New Patient Registrationregister_now

Patients requesting to join the practice list will be required to complete an application form and a new patient medical questionnaire. Patients are also requested to provide two forms of ID, one of which should be photographic such as a passport or driving licence, the other being proof of residence such as a recent utility bill.

As a guide, the practice covers the whole of Uddingston, Bothwell and Viewpark.

Patients on repeat medication are required to make an appointment for a new patient medical with the health care assistant and a GP. Patients with no current medical problems should make an appointment with the health care assistant.
